Various Types of Total & Partial Knee Replacement Surgery

Total Knee Replacement1. Total Knee Replacement or TKR

TKR is the answer to relieve painful knee joints. Increased wear and tear of the joint surfaces and cartilage due to increasing age or overuse causes extreme pain that may prevent a person from walking more than a few steps. TKR resolves pain and increases the mobility of the person.

Osteoarthritis, ligament, meniscal and cartilage tears are the most common causes of pain and disability in the knee. TKR or Total Knee Arthroplasty can help resolve all these problems and increase the quality of life of the patient. In this surgery the degenerated joint surfaces of the lower end of thigh bone, upper end of tibia and the patella are scraped off and new artificial joint surfaces are fitted in. The metal or plastic implant is fixed with the help of biological cement. These implants have a long life of 10-15 years. Physiotherapy rehabilitation after surgery is extremely important to teach the patient how to use the new knee joint.

2. Partial Knee ReplacementTotal Knee Replacement

Partial Knee Replacement involves replacing only one part of the knee joint with the implant. Knee joint is anatomically divided into three compartments- medial (inner)tibio femoral joint, lateral (outer) tibio femoral joint and patella femoral joint. Most of the time the degenerative changes associated with the arthritis are restricted to only one of these compartments, which makes changing the whole knee joint totally unnecessary. In such cases only the unhealthy knee surfaces are cut and replaced by the metallic or plastic implant. The benefit of this procedure is shorter hospital stay and faster rehabilitation.

Replacing the whole joint in these patients is overkill and unnecessary. Many middle aged men and women develop osteoarthritis of the knee. Osteoarthritis of the knee affects the inner half or medial compartment to start with and then proceeds to affect the outer half or lateral compartment. In this operation only that part of the knee, which is unhealthy, is replaced. The normal surfaces are left alone. This operation has several advantages over total knee replacement surgery like small incision, longer life of implant and post operative hospitalization is reduced and return to normal is much faster than total knee replacement surgery.

3. Simultaneous Bilateral Knee Replacement- Both Knee Replacement Surgery Total Knee Replacement

Simultaneous bilateral knee replacement is undertaken when both knees are badly affected by osteoarthritis and cannot be treated conservatively. Both the knee joints of the patient are replaced with implants in a single session of hospitalisation. This cuts down on re admission costs.

4. Revision Knee Replacement

Revision knee replacements are undertaken after 10-15 years as the life of an implant is considered around 15 years. In case the person has undergone total knee replacement early in his life he may require a repeat surgery to replace the old implant with a new one.

5. Arthroscopic Surgery of the Knee

This is a minimally invasive approach used to visualise the inside of the knee joint and also repair and reconstruct torn cartilage, damaged ligaments and meniscus. This surgery requires only 2-3 small holes or ports through which micro instruments are inserted to do the repair. An arthroscope is a fine tube with a light and camera at the end. The camera sends real time images of the inside of the joint during the surgery which can be seen on the monitor. This helps the surgeon to navigate the instruments during surgery. This surgery ensures smaller scars, lesser blood loss, lesser collateral damage and hence faster recovery.

  • ACL Reconstruction is the repair of anterior cruciate ligament that keeps the leg bone attached to the thigh bone during straightening of the knee. This ligament is commonly torn in contact sports such as football, basketball, soccer etc. ACL reconstruction is one of the surgeries performed by arthroscopy.
  • Arthroscopic Menisectomy menisci are small semi-circular moon shaped structures on the surface of the tibia (leg bone). It is responsible for the good fit of the lower surface of the thigh bone on the upper surface of the leg bone. It also enables smooth movement of the knee joint. Sudden twisting while sitting, standing or playing can result in tear of the meniscus causing extreme pain and disability. Trimming of the torn portion of the meniscus or removal of the loose meniscal end is also performed through arthroscopy using micro instruments and absorbable sutures. Generally the repair heals faster in younger patients as compared to older patients.
