Sinusitis is the commonest nasal ailment affecting large number of people.The four pairs of sinuses located in the skull – frontal (in the forehead), maxillary (in the cheek bones), sphenoid (behind the eyes) and ethmoid (between the eyes and behind the nasal bridge); function primarily to keep the skull light and allow ventilation. The sinuses are lined by a thin mucus membrane that secretes a small amount of mucus to keep the nasal passages moist and trap irritants. But at times there may be excessive mucus production or mucus doesn’t drain out due to some reason and gets collected in the sinus passages. This affects normal air entry. Bacteria also thrive in such conditions causing infection.
Headache, facial tenderness to touch, fever, stuffy nose, recurrent nasal discharge etc. make it difficult for patients to enjoy their normal routine. In chronic sinusitis the symptoms last for more than 12 weeks and surgical treatment is required. Chronic sinusitis occurs because of nasal polyps and deviated nasal septum. Presence of either of these affects the drainage of mucus from the sinuses. As a result the mucus gets accumulated making it a perfect place for bacterial infection. Surgery is necessary to remove the polyps and correct the septum.
In earlier days, sinus surgery involved making cuts in the nose, face and mouth to reach the sinuses. The incisions were painful and slow to recover. The scars remained for a long time. The advent of laparoscopic surgery has revolutionised nasal sinus surgery in India.
FESS is much less invasive than older conventional surgical methods. But because the extent of sinus disease varies from person to person, surgery may be a relatively minimal procedure or an extensive and prolonged operation. With FESS there is very little postoperative discomfort – and an excellent chance of improvement in symptoms. It has become a common procedure, but a background check of the surgeon is advisable.
Fiberoptic telescopes are used for diagnosis and during the procedure, and computed tomography is used to assess the anatomy and identify diseased areas. Functional endoscopic sinus surgery should be reserved for use in patients in whom medical treatment has failed. The procedure can be performed under general or local anesthesia on an outpatient basis, and patients usually experience minimal discomfort. The complication rate for this procedure is lower than that for conventional sinus surgery.
This surgical procedure often greatly improves the function of the osteomeatal complex and provides better ventilation of the sinuses. Many new refinements have also been made with Functional Endoscopic Sinus Surgery (FESS) surgery. Nowadays, doctors are doing their operations through a video attachment to the endoscope that allows surgeons to look at a video monitor. It is less of a physical strain.
Balloon sinuplasty which is similar to balloon angioplasty can also be performed along with this surgery. A stent with a balloon is inserted through the endoscope and inflated at the site of nasal occlusion. This can widen the sinus channels.
