In vitro fertilization (IVF) is a series of procedures used to treat fertility with the conception of a child. IVF means fertilization of egg and sperm outside the body in a test tube. During IVF, mature eggs are collected (retrieved) from the ovaries and fertilized by sperm in a lab. Then the fertilized eggs are implanted into the uterus. One cycle of IVF takes about two weeks.
IVF is the most effective form of assisted reproductive technology. The procedure can be done using one’s own eggs and partner’s sperm. Or IVF may involve eggs, sperm or embryos from a known or anonymous donor.
If more than one embryo is implanted in the uterus, IVF can result in a pregnancy with more than one fetus (multiple pregnancy).
An IVF cycle consists of ovarian stimulation, egg collection, fertilization, embryo culture and embryo transfer. Ovarian stimulation consists of daily injection of hormones to the female partner with frequent clinical and ultrasonographic monitoring. This usually continues for 10-15 days. Egg retrieval is performed under anaesthesia through the internal route. There are no incisions or scars.
The egg and sperm are either mixed (IVF) or the sperm is injected into the egg (ICSI-intracytoplasmic sperm injection).
The fertilized egg forms the embryo. The embryo is cultured in the laboratory for 2-3 days . Two-three best embryos are selected and transferred into the womb (uterus) on the second or the third day. The pregnancy test is done 14 days after the day of embryo transfer.
Intrauterine insemination – In (IUI) warmed prepared sperm is introduced into the woman’s uterus (womb) at around the time of ovulation, with the aim of getting the sperm nearer to the egg. The procedure is often combined with infertility medicines. IUI helps couples where the man has a low sperm count or poor motility (the ability of the sperm to move), as long as there are sufficient levels of healthy, motile sperm to make the treatment worthwhile. If not, IVF or ICSI will be more suitable. Different clinics use different criteria to decide whether the quality of sperm is sufficient for IUI.
Because sperm is placed directly inside the woman, IUI can also help couples who are unable to have intercourse because of disability, injury, or difficulties such as premature ejaculation. It is also recommended for women with mild endometriosis, and is often used as the first line of assisted conception treatment for couples with “unexplained infertility”. Women without a male partner may also want to have IUI, as using sperm donated through a clinic has a number of advantages over self-insemination at home.
