No widely accepted definition of what is an abnormally small penis exists, but guidelines for selection of men for penis enlargement (penile augmentation surgery) recommend that only those with a flaccid length of less than 1.6 inches (4.1cms) ) or an erect length of less than 3 inches (7.6cms) should be considered for surgery.

As part of complete penis enlargement surgery, the penile lengthening procedure essentially exteriorises the part of the penis that is hidden inside the body, which makes for about one third of the total penis length. Penile lengthening is done by cutting the ligaments (suspensory ligaments) at the base of the penis via a low abdominal incision. These ligaments act as the “guy ropes of the tent” and, once severed, allow the penis to lengthen naturally. Tissue is then placed between the pubic bone and the lengthened penis. The drawback of this surgery is that the penis generally faces upwards during erection and the position is supported by these ligaments. Once the ligaments are cut the erect penis may face downwards.
2. Girth Enlargement
To widen the penis during penis enlargement surgery, dermal fat grafts taken from the stomach or buttocks to widen the erectile cylinders of the penis underneath the skin. This ensures the increase in diameter is even all along the shaft and back inside the tissues deep within the intra-pubic region. Some surgeries involved injecting fat under the skin covering the penis. This is not reliable on long term basis, as the fat may dissolve unevenly leaving a misshapen (cobble shaped penis). Skin grafts are more reliable than fat grafts.
Associated Risks and Postoperative care
